This role is pivotal in assisting our US-based providers in managing patient care efficiently.

**Job Description:**
The Remote Care Coordinator will work directly under the supervision of our healthcare providers and will be instrumental in managing various administrative and clinical tasks. This position requires effective communication skills, a high level of organization, and a strong foundation in medical terminology and chronic health conditions.

**Key Responsibilities:**
– Schedule and coordinate appointments for patients using our electronic systems.
– Create and maintain patient charts with high accuracy in our electronic medical record (EMR) system.
– Utilize a VOIP phone system to communicate with US-based patients, ensuring clarity and professionalism in all interactions.
– Understand and apply basic medical terminology and concepts related to chronic health conditions.
– Facilitate communication between patients, care team members, and providers to ensure cohesive care delivery.
– Manage time effectively to handle a variety of tasks simultaneously without compromising the quality of work.
– Use Microsoft Teams for internal communication and coordination with the healthcare team.
– Engage in critical thinking to solve problems and make informed decisions that benefit patient care.
– Provide basic technical support to an elderly patient population, assisting them with using technology to access healthcare services.

**Qualifications:**
– Medical degree from a recognized institution.
– Fluent in English, both verbal and written.
– Strong background in clinical medicine, preferably with experience in managing chronic conditions.
– Proficient in using electronic medical records and other healthcare management systems.
– Excellent communication and interpersonal skills, with the ability to interact effectively with a diverse patient base and healthcare team.
– Strong organizational skills and the ability to manage multiple tasks efficiently.
– Experience in using Microsoft Teams and other communication tools.
– Ability to provide remote technical support and guide patients in using digital health tools.

**Working Conditions:**
– This is a remote position requiring a dedicated home office space free of distractions.
– Regular working hours align with US Central Time (CST) to coincide with clinic hours in Houston, TX.
– This role may require occasional availability during evenings and weekends to meet the needs of our patients.
