**Job Description: Care Coordinator Assistant**
**Position Overview:**
We are seeking a detail-oriented and compassionate Care Coordinator Assistant to join our healthcare team. As a Care Coordinator Assistant, you will play a crucial role in ensuring seamless patient care and administrative support. Your responsibilities will include updating patient charts and schedules, creating new patient charts, performing welcoming calls to enhance the patient experience, and contributing to the team’s success.
**Responsibilities:**
1. **Patient Chart Management:**
– Update and maintain electronic patient charts with accurate and current information.
– Ensure all medical records, test results, and documentation are appropriately filed within patient charts.
– Collaborate with the care coordination team to ensure patient records are up-to-date and easily accessible.
2. **Scheduling:**
– Assist in managing and updating the patient schedule in accordance with physician appointments, treatments, and follow-up visits.
– Coordinate with healthcare providers and staff to schedule appointments, consultations, and procedures for patients.
– Communicate any schedule changes or conflicts promptly to the care coordination team.
3. **New Patient Chart Creation:**
– Create new electronic patient charts accurately and efficiently, ensuring all necessary information is included.
– Collect and input patient demographics, medical history, and insurance details into the electronic health records system.
– Verify insurance information and facilitate the authorization process as needed.
4. **Welcoming Calls and Communication:**
– Initiate welcoming calls to new patients, providing them with essential information about upcoming appointments, procedures, and what to expect during their visit.
– Address patient inquiries, concerns, and provide guidance on pre-visit instructions or preparations.
– Establish a positive and reassuring rapport with patients, promoting a welcoming and patient-centered experience.
5. **Administrative Support:**
– Assist in maintaining and organizing administrative documents, forms, and communication related to patient care.
– Collaborate with front desk and reception staff to ensure a smooth check-in and check-out process for patients.
– Participate in team meetings and contribute to discussions on process improvement and patient care enhancement.
**Qualifications:**
– some coursework in healthcare administration or related field is a plus.
– Strong organizational skills and attention to detail.
– Excellent interpersonal and communication skills.
– Proficiency in using electronic health records (EHR) systems and Google Office Suite.
– Ability to handle sensitive patient information with confidentiality and discretion.
– Prior experience in a healthcare or administrative role is preferred but not required.
