Responsibilities:
• Engage with a vibrant and dynamic team remotely, working for a well-respected company serving the MD/DC Metro area.
• Conduct virtual new and prospective client meetings to assess service needs, with the option for in-person meetings based on client requests.
• Perform remote wellness checks and host virtual informational wellness sessions for CWC clients, caregivers, and other interested members.
• Develop and update a Plan of Care for all new clients, tailored to their unique needs.
• Maintain seamless communication with care managers, caregivers, office staff, clients, family members, and other medical professionals to address clients’ needs and concerns.
• Collaborate with the Director of Nursing for the supervision of services and consult on client care plans.
• Ensure the proper maintenance of clinical records and documentation in compliance with State and Federal laws, leveraging digital tools and platforms.
• Conduct client and caregiver evaluations remotely, as needed, to ensure the highest quality of care.
• Provide virtual training to both new and existing staff on care protocols and procedures.

Qualifications:
• RN License
• Exceptional ability to adapt to digital tools and remote communication platforms.
• Strong organizational skills and the ability to manage documentation and compliance remotely.
