Job Description

SPECIFIC DUTIES & RESPONSIBILITIES
A. Service to employees:
1. Assist Recruitment section in processing of medical examinations of applicants by:
1.1 Issue medical referral to those who have passed the due process of screening and examination.
1.2 Check and assess medical result of applicants and sign their 201 File (application form) indicating whether the applicant has passed or failed the medical examination.
1.3 Filing of pre-employment medical records.
1.4 Conduct Pregnancy Test to female applicants for schedule of orientation.
2. Render basic health counselling individually upon personal consultation of employees.
3. Review periodically the health status of employees with chronic ailments.
4. Ensure prompt isolation and treatment of employees suffering from infectious or communicable diseases on the first notification to protect the health of the rest of the employees in the work area.
5. Coordinate with Safety section to detect early hazards to employees’ health and minimize them.
6. Prompt reporting to Immediate Superiors of unusual incidents or complaints that may threaten the health or safety of employees.
7. Provide employees with information on health care, family planning, or other medical-related matters through informative materials (e.g., brochures, pamphlets, handouts).
8. Prepare Annual Physical Examination schedule and submit to Superior for approval.
9. Distribute medical result to employees and give necessary advice regarding their health condition.
10. Issue referrals to employees with medical findings and those who need further medical evaluation/management and follow-up their condition thereafter.
11. Establish health programs such as conducting lectures, seminars, and counselling which can help promote wellness of the working population.
B. Services to ill employees
1. Assess patient’s health condition and apply the appropriate nursing interventions.
2. Endorse to company physician for thorough check-up (if on-duty).
3. Explain to patients the proper frequency and dosage of medicines prescribed by the company physician, and administers initial dosage if available.
4. If employee is unfit to work, issue sick employee gate pass and ensure patient’s safety.
5. Issue leave form to sick employee and check details before sending to concerned department.
6. If company physician is not available and the sick employee needs to see a doctor, issue medical referral for free check up in GT in case of contractual employee, or advice the member in case of regular.
7. Give necessary advice to employee about absences rules in case the doctor gives rest days or continuous absences.
8. In case the employee needs to be taken to the hospital, assist the employee and notify the relatives.
C. Services to injured employees
1. Assess patient’s condition and give necessary first aid treatment.
2. Endorse to company physician for further medication/treatment (if available).
3. Accompany patients to hospital if necessary and notify the relatives in case of confinement.
4. Inquire expenses in case the patient needs operation.
5. Request approval to Admin Superior regarding patient’s operation.
6. Coordinate to GT Hospital and relatives regarding patient’s operation or condition.
7. Give updates to superior regarding patient’s condition.
Others:
8. Perform abovementioned duties during company activities such as summer outing, sports festival, Christmas party, and other company-related activities.
9. Extend nursing service by conducting home visit to employees with serious medical condition or prolong leave of absence due to sickness.
10. Assist patients during follow up check up until fully recovered when necessary (Company related incidents).
11. Accomplish/Issue Authority/Referral Letter for employees seeking medical, laboratory, x-ray or dental services with accredited/non-accredited hospitals/clinics centers.
12. Close monitoring of patients under maintenance therapy due to communicable diseases and monitor medicine supply of patient for the whole duration of treatment.
13. Issue and submit to Accounting-Benefits section the patient’s SSS Sickness Notification and ECC in case of industrial accident.
D. Administrative
1. Maintain the employee’s individual medical records and other clinic records.
2. Issue, check, and verify supporting documents for filing medical benefits (e.g. Maternity 1, Sickness Notification, and ECC forms).
3. Check validity of details, supporting documents for filing emergency loan in case of calamities, sickness, or deaths (e.g. medical certificate, birth certificate, death certificate, prescriptions, etc.).
4. Review and check statement of accounts submitted by the company’s accredited medical clinics and hospitals.
5. Proper recording of issuance or medicines and other supplies.
6. Accomplish monthly clinic reports (e.g. Accident, Medical, Dental, Medicine Inventory) then submit to Administration Adviser for signatories.
7. Ensure adequate availability of supply and requisitions for replenishment in case of shortage of clinic forms, medicines, ambulance supplies and medicines.
8. Maintains inventory of clinic equipment and ensures sterility of instruments.
9. Issue Fit to Work clearance to members.
10. Perform other tasks that may be assigned by the superiors from time to time.
