What is a Health Coordinator? The Health Coordinator is responsible for the coordination of navigation activities based on a given assignment of service types and works hand-in-hand with peer case coordinators and staff to provide quality customer care. The Health Coordinator facilitates communication between interdisciplinary members and expedites customer services. The Health Coordinator is dedicated to providing support to customers and managing customer cases but is not responsible for any type of clinical care.

Qualifications:
Commitment to the mission of care coordination
> Strong communication and interpersonal skills and ability to speak concisely to customers, vendors, physicians, and care team members
> Strong customer service skills
> Attention to detail and good ability to handle multiple tasks and requests
> Organized with confidential client material, appointment tracking, and caseloads
> Ability to build relationships with customers, work colleagues & vendors
> Committed, responsible, and abides by high ethical and moral standards

Education and Experience:
> Strong preference for Registered Nurse, EMT / EMD
> 3 years of minimum of case management, customer service, or healthcare setting experience
> Strong understanding of cultural competency with the target population
> Experience helping people with health problems – either as employee or fulfilling family obligations
> Computer literacy necessary
> Organizational, problem solving and time management skills
> In respect of confidential medical information of members, candidate must be willing to deliver services in Navigate Global’s physical office on shifting schedule
