We are searching for a compassionate and meticulous individual to become a part of our Hospice Intake/Quality Assurance (QA) team. The role of an Intake and Hospice QA is vital in ensuring the highest standards of care and compliance provided by the Hospice Agency.

Intake/QA work schedule: Monday – Friday 8:00am – 5:00pm PST

Responsibilities:
[INTAKE]
• Check patient details of patients referred and request medical records from the hospital where the patient is admitted.
• Assess patient eligibility for hospice care, and complete the admission process.
• Serves as a point of contact for patients and their families, providing information about hospice services, answering questions, and addressing concerns. Offers emotional support and guidance throughout the intake process.
• Verifies insurance coverage for hospice services, assists with obtaining authorization, and explains the financial aspects of hospice care to patients and families.
• Collaborates with the interdisciplinary team, which may include nurses, doctors, social workers, and chaplains, to develop a comprehensive care plan for each patient and ensure that the team is informed about the patient’s admission and any specific needs.
• Ordering medications of patients, coordinating with hospice medical director/doctor on processing medication reconciliation or changes on medication that patient is taking.
• Coordinate with the facility where the patient is residing to inform or send them changes in the patient’s plan of care.

[QA]
• Perform regular audits of patient charts, care plans, and documentation to ensure accuracy, completeness, and adherence to regulatory guidelines.
• Collaborate with the interdisciplinary team to develop and implement performance improvement initiatives based on identified areas of improvement.
• Provide education and training to staff regarding quality improvement principles, standards of care, and regulatory requirements.
• Review and analyze data related to clinical outcomes, patient satisfaction, and compliance metrics to identify trends, patterns, and areas for improvement.
• Participate in interdisciplinary team meetings to provide insights and recommendations for enhancing the quality of care provided to hospice patients.
• Act as a resource for staff members, providing guidance and support in maintaining quality standards and resolving quality-related issues.

Qualifications:
• Nursing Graduate [FRESH GRADUATES ONLY]
• Excellent analytical and problem-solving skills.
• Effective written and verbal communication skills.
• Ability to collaborate and work well within a team environment.
• Detail-oriented and able to maintain accurate records and documentation.
• Commitment to providing compassionate and patient-centered care.
