We are seeking a highly organized and detail-oriented Intake/Scheduling Coordinator to join our Home Health Services team. The ideal candidate will play a crucial role in managing the intake process and coordinating scheduling activities to ensure efficient and timely delivery of home health services.

Responsibilities:
Coordination:
• Receive and process incoming referrals from healthcare providers, physicians, and other sources.
• Gather necessary information for patient intake, including demographics, insurance details, and medical history.
• Verify insurance coverage and obtain authorizations as required.

:
• Coordinate and schedule home health visits for patients based on their needs and the availability of healthcare providers.
• Communicate with patients, caregivers, and healthcare providers to confirm appointment details.
• Ensure accurate and up-to-date scheduling information in the system.

:
• Serve as the primary point of contact for inquiries related to scheduling and intake.
• Collaborate with healthcare providers, nurses, and other team members to address scheduling challenges and ensure optimal patient care.

:
• Maintain accurate and complete records of patient information, including intake forms, scheduling details, and insurance
documentation.
• Prepare and submit reports on intake and scheduling activities as needed.

Assurance:
• Monitor and assess the efficiency of the intake and scheduling processes, identifying areas for improvement.
• Participate in quality assurance initiatives to enhance overall service delivery.

Qualifications:
• Proven experience in a similar role within a healthcare setting, preferably in home health.
• Strong organizational and multitasking skills with meticulous attention to detail.
• Excellent communication and interpersonal abilities.
• Proficient in using scheduling software and electronic health records systems.
• Knowledge of healthcare regulations and insurance processes is an advantage.
