Our mission is to guide individuals through the journey of finding and securing safe, stable homes. We provide essential services including financial assistance for initial housing costs and ongoing support to maintain stable living situations. Additionally, we offer temporary residential care for those with medical or behavioral health needs but without stable housing. Employing a scattered site housing model, Rooted Life ensures each residence is fully furnished, creating environments conducive to healing and recovery. What You’ll Be Doing:
???? Vital Support and Liaison:Acting as a vital support and liaison for care coordinators.
???? Comprehensive Researcher:Performing comprehensive online research to identify local resources and programs in California.
???? Dedicated Communicator: Making numerous phone calls to ensure resource availability and appropriateness for clients.
???? Resource Advisor: Recommending suitable and accessible resources for individual client needs.
???? Appointment Coordinator: Scheduling and managing client appointments, ensuring continuity of care.
???? Meticulous Record Keeper: Maintaining detailed records and files for clients and staff.
????? Team Communicator: Ensuring effective communication within the team to maintain a unified approach to client care

This position is key to our mission at Rooted Life, serving as a crucial link between our clients and the resources they require. The ideal candidate is proactive, empathetic, and possesses a strong research background. They will play an integral role in understanding and navigating the complexities of California’s housing and healthcare systems, making a substantial difference in the lives of individuals facing significant challenges. This role offers a rewarding opportunity to contribute meaningfully to the wellbeing and stability of our clients.

Qualifications & Requirements:
University Degree in Social Work, Nursing, or similar health sciences degree from a recognized university
2+ years experience in research, communication, and coordination, ideally in healthcare or social work.
Strong command of written and spoken American English
Adaptable to new information and settings
Must have 50 Mbps or more download speeds, and at least 10 Mbps upload speeds
This is a remote position, you must be comfortable working online and have a quiet home or office space to work from
