Job Description
We are seeking a Remote Case Management Social Worker Assistant to join our dynamic team. This position is vital in facilitating effective communication with our clients and managing essential paperwork to ensure seamless service delivery. If you are organized, empathetic, and have a passion for helping others, this role is for you.
Key Responsibilities
1. Communicate effectively with elders we serve, family members, government agencies, and Healthcare providers, and understand their needs and provide timely responses.
2. Assist in the coordination of care by scheduling appointments, arranging transportation, and following up on services.
3. Maintain accurate and up-to-date records of client interactions, services provided, and case progress.
4. Assist in the preparation and filing of necessary paperwork and documentation in compliance with Medi-Cal regulations, California Residential Care Facility for the Elderly regulations, and California Healthcare regulations.
5. Collaborate with case managers, healthcare providers, and other team members to ensure a holistic approach to participant care.
6. Participate in team meetings and training sessions to enhance service delivery and personal professional development.
7. Other relevant tasks to ensure proper documentation and service delivery for our elders in care.
Qualifications
1. Strong verbal and written communication skills in English.
2. Proficiency in using digital communication tools and case management software.
3. Detail-oriented with excellent organizational and multitasking abilities.
4. Experience in a customer service or administrative role, preferably in a healthcare or social services environment.
5. Understanding of Medi-Cal policies and procedures is an asset.
6. Empathetic and patient-centered approach to client interactions.
7. Ability to work independently and as part of a remote team.
