Rooted Life is a compassionate and innovative organization operating in California, dedicated to aiding those experiencing homelessness, recovering from hospital stays, or facing housing challenges. Our mission is to guide individuals through the journey of finding and securing safe, stable homes. We provide essential services including financial assistance for initial housing costs and ongoing support to maintain stable living situations. Additionally, we offer temporary residential care for those with medical or behavioral health needs but without stable housing. Employing a scattered site housing model, Rooted Life ensures each residence is fully furnished, creating environments conducive to healing and recovery. What You’ll Be Doing:
You will work closely with referrals to assess their needs and ensure a smooth transition into our various programs. This position plays a critical role in both helping individuals access the support they need and revenue management
???? Customer Service Excellence: Providing the highest level of service to both internal and external customers in a fast-paced, team-oriented environment.
???? Active Intake Calls: Making the assigned number of intake calls and collaborating with the operations manager to reach team goals
???? Referral Process Management: Managing the total referral process, including initiation, patient, and insurance verification, as well as assignment of referrals and record-keeping.
???? Service Determination: Determining services that referrals may require.
???? Client Connection: Ensuring clients are connected to appropriate staff and the services necessary to meet their needs.
???? Seamless Referral Collaboration: Collaborating with other staff and community service providers to ensure seamless referral services.
?? Efficient Prioritization: Prioritizing and working efficiently throughout the workday with a strong attention to detail.
hContinuous Workflow Improvement: You are expected to be an expert in Intake workflows and
Qualifications & Requirements:
2 years minimum experience as an Intake Coordinator
Strong command of American English and relatively neutral accent
Strong organizational, communication, and interpersonal skills.
Proficiency in computer applications and databases.
Excellent data entry and typing skills
