Responsibilities
•
Job Description: The Patient Care Coordinator manages the nurse/caregiver/client relationship
•
The Patient Care Coordinator is responsible for understanding the staffing needs of each client family to understand the client’s staffing preferences
•
The Patient Care Coordinator is also responsible for managing and developing relationships with our caregivers and nurses
•
The Patient Care Coordinator must work closely with our caregivers and nurses to understand their staffing desires and limitations to best match our nurses with our clients
•
The Patient Care Coordinator must provide exceptional customer service to both our clients and our nurses
•
In addition to managing our nurses and providing excellent customer services to our clients, the Patient Care Coordinator is responsible for posting ads, recruiting, interview, and hiring / reactivating nurses to join our healthcare field staff
•
The Patient Care Coordinator will work closely with our clinical supervisors to manage our healthcare field staff and ensure all employees are appropriately following company policies & procedures
•
Consistently provides excellent customer service to our patients and their families
•
Actively Posts ads online and on job boards
•
Recruits and conducts Interviews for new caregivers/nurses for open positions
•
Posts nurses’ notes into the Electronic Medical Records System
•
Calls families to schedule nurses/caregivers
•
Calls caregivers to schedule visits
•
Conduct patients meet and greets and admissions
•
Conducts sales calls to grow patient census

May work some weekends and holidays as needed
