Job Description

Using telephone communications, you evaluate each client while create care plans, discuss patient goals and health concerns , and identify needs of resources.

Your responsibilities include overseeing outstanding patient care while working alongside patients, their families, and other medical professionals.

Stable internet and ability to have a VPN is important to download the phone software.

This is an overnight position typically from 10am-2pm (eastern USA time).

Preferred Educational Background:
Nurse, Medical/Nursing Assistant, EMT

Telephonic Case Management Role:
The role would be make outreach calls to the patients by following an outlined script outline of assessment questions.
Also performing or updating the patient assessment and plan of care.

Great communication skills, timely and detailed documentation and empathy towards the patients.

Many of the patients are elderly with multiple disease processes, so it’s important to be knowledgeable in chronic conditions like Diabetes, COPD, CHF, HTN, Cancer and Depression.

Technology:
We use Google Suite to share information between the team and have a final review before transferring into the Provider EMR.

We use google voice to communicate with patients, so a strong internet signal is very important.

We also use an application called Trello, where we document all the tasks associated with the patients we serve.

The goal is to be an additional support for the physician and identify any issues to be reported to them.

Case Management Scenario:

Ms. G is a 67 year old woman is diagnosed with diabetes and heart failure. She has been hospitalized multiple times due to her diabetes and it is out of control. She forgets to take her medications and cannot afford her insulin. She is gaining weight due to mostly eating frozen meals. She never saw the endocrinologist that was recommended and she lives at home alone and has trouble getting to the doctors.
