Position Overview:
The Home Health Case Manager is responsible for coordinating and managing the care of patients in their homes. They work
closely with healthcare professionals, patients, and their families to develop and implement personalized care plans. The Case
Manager ensures the delivery of high-quality healthcare services and facilitates effective communication among all involved
parties.

Key Responsibilities:

1. Care Coordination: Assess patients’ needs and develop comprehensive care plans that meet their specific requirements.
Coordinate with healthcare professionals, including physicians, nurses, therapists, and social workers, to ensure the appropriate delivery of services.

2. Patient Advocacy: Serve as a patient advocate, ensuring their rights are protected and their needs are met. Collaborate with
patients and their families to establish goals and promote patient engagement in their care.

3. Assessment and Monitoring: Conduct thorough assessments of patients’ health conditions, functional abilities, and support
systems. Regularly monitor patients’ progress, update care plans accordingly, and communicate changes to the healthcare
team.

4. Care Plan Implementation: Coordinate and oversee the implementation of care plans, including arranging for home health
services, medical equipment, medications, and therapy sessions. Ensure that all services are delivered timely and accurately.

5. Communication and Collaboration: Facilitate effective communication among healthcare professionals, patients, and their
families. Serve as a liaison between patients and the healthcare team, ensuring the timely exchange of information and
addressing any concerns or challenges that may arise.

6. Documentation and Reporting: Maintain accurate and up-to-date documentation of patient assessments, care plans,
interventions, and progress. Prepare reports and communicate information as required by regulatory bodies, healthcare
agencies, and insurance providers.

7. Quality Assurance: Monitor and evaluate the quality and effectiveness of care provided to patients. Identify areas for
improvement and implement appropriate measures to enhance patient outcomes and satisfaction.
8. Compliance: Ensure compliance with all regulatory guidelines, industry standards, and organizational policies. Adhere to
legal and ethical standards of practice.
